如多數傳染病病人在有臨床癥狀時能排出大量病原體，威脅周圍人群，是重要的傳染源。但有些病人如百日咳患者，在卡他期排出病原體較多，具有很強的傳染性，而在痙咳期排出病原體的數量明顯減少，傳染性也逐漸減退。又如，乙型肝炎病人在潛伏期末才具有傳染性。
一般說來，病人在恢復期不再是傳染源，但某些傳染病（傷寒、白喉）的恢復期病人仍可在一定時間內排出病原體，繼續起傳染源的作用。
病原攜帶者指已無任何臨床癥狀，但能排出病原體的人或動物。攜帶者有病后攜帶者和所謂健康攜帶者兩種。前者指臨床癥狀消病毒、機體功能恢復，但繼續排出病原體的個體。這種攜帶狀態一般持續時間較短，少數個體攜帶時間較長，個別的可延續多年，如慢性傷寒帶菌者。所謂健康攜帶者無疾病既往史，但用檢驗方法可查明其排出物帶病原體。這種人攜帶病原體的時間一般是短暫的。
病動物也是人類傳染病的傳染源。人被患病動物(如狂犬病、鼠咬熱病獸)咬傷或接觸病動物的排泄物、分泌物而被感染。
人和動物可患同一種病，但病理改變、臨床表現和作為傳染源的意義不相同。如患狂犬病的狗可出現攻擊人和其他動物的行為，成為該病的傳染源之一，而人患此病后臨床表現為恐水癥，不再成為該病的傳染源。
傳播途徑 指病原體自傳染源排出后，在傳染給另一易感者之前在外界環境中所行經的途徑。一種傳染病的傳播途徑可以是單一的，也可以是多個的。傳播途徑可分為水平傳播和垂直傳播兩類。
由于生物性的致病原于人體外可存活的時間不一，存在人體內的位置、活動方式都有不同，都影響了一個感染癥如何傳染的過程。為了生存和繁衍，這類病原性的微生物必須具備可傳染的性質，每一種傳染性的病原通常都有特定的傳播方式，例如透過呼吸的路徑，某些細菌或病毒可以引起宿主呼吸道表面黏膜層的型態變病毒，刺激神經反射而引起咳嗽或噴嚏等癥狀，藉此重回空氣等待下一個宿主將其入，但也有部分微生物則是引起消病毒系統異常，像是腹瀉或嘔吐，并隨著排出物散布在各處。透過這些方式，復制的病原隨患者的活動范圍可大量散播。
